Cardiology: Pacemakers Pacemaker implantation information in companion animals was published in the Journal of Veterinary Cardiology, by TVEH resident cardiologist, Dr. J Lichtenberger, with co-authors KF Scollan, BJ Bulmer & DD Sisson. Excerpts provided below… Articial pacemakers: Small devices that function by delivering small electrical impulses to the myocardium at predetermined minimal heart rate. Modern pacemakers provide pacing as close to the patient's natural rhythm as possible, by sensing the heart's normal activity & only pacing when required. Sensing the patient's movements, they increase the heart rate to match the activity level. Indications: Pacemaker therapy in dogs & cats is indicated in symptomatic bradyarrhythmias; the 2 most common conditions being high-grade atrio-ventricular block or sinus node dysfunc- tion (sick sinus syndrome). Common clinical signs include syncopal episodes & lethargy. Implantation: Dogs: Minimally invasive approach via the jugular vein. Pacing lead is placed in right ventricle using uoroscopic guidance & generator is placed SubQ in cervical region. Cats: Usually placed on epicardial surface of heart, requiring a transdiaphragmatic approach. At TVEH, we employ the brand new Dextronix system from Biotronik that carries a 10-year battery life. Aercare: Exercise restriction: Recommended for 1 month post- implantation. Setting is then readjusted using a programmer. Every 6-12 months: Rechecks recommended. The success rate of pacemaker implantation is high and prognosis is usually excellent in uncomplicated cases.
